Global adjustments to all Output/Report layouts

This applies to all outputs including NOA, Inc Application, Annual Report, Director & Address Change plus any new ones that have been created.

Refer to design links and the attached screenshots

1. Increase the size of Carol's signature graphic and centre it vertically to match more closely with the Design Comp: https://projects.invisionapp.com/share/8SWB65GZA6J#/screens/425964628/comments
2. Make the business details/signature section a common template for all outputs (so if we need to update labels or the signature in the future this can be done easily across all outputs)
3. Insure there is a minimum of 30px of white space at the bottom of each page between the end of the page content and the footer logo
4. For outputs that include the Share Structure chart;
A) adjust column widths by increasing the column widths for Max Number of Shares and Par Value to allow for longer share numbers and par values. This may mean reducing some of the gutter widths between columns (keep a minimum of 20px gutters between each column) and/or reducing the width of the name column. B) test and insure proper wrapping of long names as well as long worst case scenario Share numbers and values. (refer to design link above)
C) Par Value should show minimum of 2 decimal points. ie $1.00 not $1.0 or $1

5. Make sure minimum space above and below the content within the light blue area is the same (refer to Design comp --> https://projects.invisionapp.com/share/8SWB65GZA6J#/screens/425964628/comments)
6. Inc Application test output showed a large amount of empty white space at the bottom of "Completing Party", before "Completing Party Statement" started. Please resolve.
