Need to keep track of which director address is changed

## Description:
The COD output needs to identify which director's address was changed. This will require some development in the UI (to capture an action identifying which address was changed) and some development in the BE (to identify in the outputs which address was changed).

I am not sure if there are other pieces that need to be updated.
