### Changes made to the current form
#### Instructions
A new instruction was added for Item F requiring a court order number for filings made pursuant to a plan of arrangement.
An instruction for Item H was added specifying that an authorized signing authority must sign where the liquidator is a corporation or firm.
The instruction regarding liquidation records office addresses was renumbered from Item F to Item G.
The filing fee instructions were updated to remove the reference to payment in the equivalent amount of U.S. funds.

No changes were made to the contents of sections A to E
Section F – Court Order and Plan of Arrangement
A new section titled "Court Order and Plan of Arrangement" was added.
A new indicator was added to identify filings made pursuant to a plan of arrangement.
A new court order number field was added.
Section G – Liquidation Records Office
The liquidation records office section was renumbered from Section F to Section G.
Section H – Certification
The certification section was renumbered from Section G to Section H.
