Policy confirmed titles are as follows:
Output #1: Statement of Intent to Liquidate
Output #2: Liquidator Appointment
Output #3: Liquidator Cessation (or Notice of Cessation of Liquidator)
Option #4: Liquidator (or Records) Address Change
