JIRA OPS - Change of Registration producing incorrect output - both outputs produce same title

ALLMAR INTERNATIONAL when we download PDFs for change of registration, both the Amendment Document and the Change of Registration have the same title, same data, but the windows pdf file is different.

Please investigate and fix.
