MHR Forms Review: Form 13

### Changes requested:

- [x] Introduce “Reason for correction” as a dedicated section with instruction “check all that apply”. Rename from transport permit amendment.
- [x] Provide checkbox options: Cancel transport permit, include note “restores registration to prior location”. Amend transport permit. Other, with text indicating correction to current location.
- [x] Remove the “correction to location only” subsection.
- [x] Group reasons into three categories: Location correction, Ownership correction, Home description correction. Add routing text that tells users which section to complete for each category.
- [x] For Location correction, require completion of current location details including location type, civic address, land details, park name and pad, dealer or manufacturer lot, plus an additional details table. Exclude the final two legacy items from the earlier layout.
- [x] Add Ownership correction options with checkboxes for mailing address update and name correction.
- [x] Add Home description correction options such as CSA decal number, serial number, year, make, and model.
- [x] Revise instruction text to clearly state which sections to complete based on selected reasons. Keep the note about submitting additional forms if needed.
- [x] Reorder sections: 1 Submitting party, 2 Manufactured Home Registration number, 3 Registered owner names, 4 Reason for correction. Remove visible numbering artifacts in the layout.
- [x] Add a document ID field and an Instructions and Notes block near the top.
- [x] Add a clear signature section with printed name, role, and date for registered owner or agent.
