### Background
Staff reported that the dropdown alone would not give enough information, so a mandatory text field beneath it is needed to record a clear explanation that transfers into the client notice. They stated that both fields should be required to ensure complete information. Staff discussed internal notes as a separate field whose requirement level still needs confirmation. They described the purpose of the change as supporting consistent categories and clear reasoning for clients.

- [ ] Update audit history wording to match the design.
- [ ] Update the "no data" state to match the design.
- [ ] Implement enhancements for supporting documents.
- [ ] Change the review decision component to radio buttons.
- [ ] Added a free-text field for decline reason.
- [ ] Add an alert message and hide the "Upload File" button if the user is not assigned.
