BRD: Coop Continuation Name Request is missing entity type

**Describe the bug in current situation**
A Continuation In NR for a Coop does not show that it is for a Coop in the BRD.

Here are 2 comtinuation in NRs for comparison:

**Impact of this bug**

A user cannot tell, when looking at the item in the BRD, that it's for a Coop. This will mostly apply to users with many items in their BRD, although the business name might tell them what they're looking at.

Coop Continuation Ins are rare, I believe, so this will probably impact few users.

Very low.

**Steps to Reproduce**

Steps to reproduce the behavior:
1. login as a regular user
2. create a continuation in NR for a Coop
3. get the NR approved
4. see the NR in your BRD

**Actual/ observed behavior/ results**

The item is missing the entity type line below the main type description.

**Expected behavior**

Like other continuation in NRs, the entity type should display in the type description.
