UI Design: supress showing former name on COD output

Future enhancement to create a way for STAFF to supress showing former name on the COD output.

**Context:**
This came up when work was being done to force user to confirm director did not change to a new person, but rather just had a legal name change. At that time design team recommended we also show former name on COD filing to help deter this type of behavior and to also be consistent with our outputs since we were already doing this there.

So we added ability to see former name only within the active session (so when they file and pay, the former name no longer shows if they start a new COD filing).

Staff requested there be a way to surpress former name on output during these discussions.
