PROD - Error shown on create new account page for some users

Repro:
1. Login to Hosts
2. On the account picker page, click on the [Create New Account] button bottom of page
3. Fill out the form
4. Click on the [Save and Start Application] button

Actual:
In production, this causes errors sometimes
Unclear on the trigger as it works in Test

Expected:
New account is added to the team and user is redirected to the Hosts wizard
