UI - Jointly, In Concert or with Associate component

Legal has identified there should be three ways to act with another Significant Individual. These are jointly, in concert and with an associate. A number of changes are needed to support having three types.

Current:

- Update title of component from "Acting jointly or in concert" to "Held jointly, exercised jointly or in concert, or associates”
- Update "Control Type" column header to read "Control"
- Update spacing so Control information is displayed below the Control header (in screenshot above the text shares jointly is the control that should display below control type. Individual connection should have the individual name box below it)
- remove preferred name
