Design Changes:
- [ ] Added a 'Payer Name' input field to both cheque and cash options.
- [ ] Input field is optional with max 50 character limit
- [ ] Show error validation if exceeded character limit
- [ ] Split the input label: Name of Person or Organization & Address into two fields (refer to design: Entity Name & Entity Address)
- [ ] Display the above information accordingly in different RS status after a routing slip is created
