UI Design - Updates to SI Email Notification Templates to remove birthdate

In the demo Sept 17 policy advised that despite sending an email to an SI themselves, we are not authorized to include the full birth date of the SI. Updates are needed in the email design for an SI under 19, SI under 19 turning of age, and SI that is 19 or older.

At the same time policy also expressed their concern about posting a direct link to the form via the email magic link. So we are removing the direct link and keeping the learn more link. **The learn more link will now be directed to the anonymous form page with the help text expanded.**

- this takes user to the fillable form (no prepopulating of SI data)
