In MHR - Add folio field for staff on review and confirm page.

Currently in MHR, QS users can enter a Folio/reference value on the confirmation page (see attached example) for the filing types they are performing.

Staff does not have the folio field on the confirmation page and are requesting to have it, for every filing that includes the confirmation page. The folio field for staff would be displayed after the submitting party info on the confirmation page, like for QS users.

The Folio number will also have to be displayed in the outputs.

Trace how confirmation-page data reaches the outputs; done means staff can enter a folio for every filing with a confirmation page, and that value appears after submitting-party information and in the outputs.
