COOPS UI - Legal name Change outputs have blank Date of Change

Description
**Describe the bug**
Post-bob change of name filings (OTNCN) show up in our system complete with a PDF output.
This output is missing the Date of Change section.

**To Reproduce**
Steps to reproduce the behavior:
1. Go to legal DB and find a post-bob change of name
2. Bring up that coop's dashboard
3. PDF will have Date of Change label with blank beside it.
**Expected behavior**
show date from the filings table, effective date which has now been confirmed to be the correct date.
