Clicking on Download Form for Financial Institutions is redirecting to an undefined page.

**Describe the bug in current situation**
When the user files and successfully pays for credit union and upon successful approval of the NR the MBR dashboard has the option to "Download Form",clicking on it is redirecting to an 'undefined' url

Steps to reproduce the behavior:
1. Go to 'https://test.account.bcregistry.gov.bc.ca/account/596/business?accountid=596'
2. Login with BCREGA001/98900001
3. Search 'NR 8909023'
4.Click on Download Form.
5. Notice that the redirection happens to an Undefined page.
