COOPS-UI: Director Change Address panel needs a Reset button

## Description:
This requirement came from Scott.

1. On the Director Change Address panel (initial change), there needs to be a disabled Reset button at bottom left.
2. On the Director Change Address panel (edit mode), there needs to be an active Reset button at the bottom left, which resets the address to original when clicked.

**Dependencies**
None.

**Acceptance Criteria**
The button should appear and function as described above.
