Pending Review Requests on Dashboard - Affidavit and Information carries over from one request to another

**Describe the bug in current situation**
While working on OPS ticket 2276, information for a request that I had recently viewed was being displayed when reviewing another pending review request. It seems to be carrying over information from one request and displaying it for other requests if that information is missing from the latter request.

**Steps to Reproduce**
Steps to reproduce the behavior:
1. Display the Pending Review tab in Account Management on the staff dashboard.
2. Click Review for PCI Audit (no information will display for Account Administrator and Notary Information, Affidavit will not download)
3. Go Back to the Staff Dashboard
4. Click Review for JENSCOLOUR (information will display for Account Administrator and Notary Information, Affidavit will download)
5. Go Back to the Staff Dashboard
6. Click Review for PCI Audit (information from JENSCOLOUR will display for Account Administrator and Notary Information, Affidavit for JENSCOLOUR will download)

**Actual/ observed behavior/ results**
Information from last reviewed entry will display on another request is any information is missing.

**Expected behavior**
Only information for the request being reviewed will display.
