Form 03 PPR

Form 03 PPR requires the following changes. _Note, these changes must be pushed to PROD at the same time as Form 03B. When complete please provide to Megan for review and discuss with staff before having them pushed to prod_

Changes
- [ ] Remove second bullet in the INSTRUCTIONS reading "There is no filing fee to issue a new code."
- [ ] Remove 'DO NOT MAIL' wording at the bottom of the form. This is currently a manual process and electronic or mail submissions are required.
- [ ] In Section 1 Application Request above the applicant name include a field for "BC Registries Account Name" and "BC Registries Account Number".
