Request section additions

· Remove >> Contact phone number as not needed

· Shipping Address >> SUF form addresses to be used for this dropdown

· Site Contact >> Remove all three options and provide free text cell for manual name entry

· Employee ID >> can we mirror the mandatory fields from the SUF form such as Employee First Name, Employee Last Name, Employee ID, Employee IDIR and Employee Supervisor.

· Remove the “Urgency” radial buttons at the bottom so all requests are general.
