{Mat/Par, all form review} Error on Name going to Payroll

**Background:**
We have several tickets in CRM which can't be processed as the users name is neither display or legal. This means they can not get their leave/pay processed

**Steps to Reproduce:**
a good example is CRM # 251217-000056 "Error on name-Digital Mat application form"
It appears the 'employee name' is being parsed from the email, not any data set as there is the number '7" in their name
**Actual Behavior:**
Odd names are being created


**Expected Behaviour:**
legal or display name should show up instead
