[Design] Registry - Project classification

**Describe the Issue**
In the Registry, we want to ask Ministry teams to provide specific information about their project.
Looking to learn if the project is related to a mandate letter commitment, or a ministry business plan.

**Additional Context**
This will help us for executive reporting. Public and Private Cloud projects support ministries delivering value that is sometimes of high importance to the government - Mandate Letter, Ministry priority - and it is important for us to showcase the value delivered by our services.

**Acceptance Criteria**

- [ ] While filling out a project request we need to add the following Yes or No questions
- [ ] Is your project related to a mandate letter commitment?
- [ ] Is your project related to a ministry business plan?
- [ ] Identify any other question that might help classify the project as related to a government priority (Stakeholder: Jenny)
- [ ] Deliver the questions in a way that prevents bias to answer "Yes" at all costs. That can be additional explanation text mentioning these questions are exclusively for reporting purposes, or else.
- [ ] Schedule a review.
