Design and platform for survey

**I want to [functionality]**
add a survey to current Go Electric site

**So that [benefit]**
we can collect data to be able to compare the user experience on goelectricbc.ca before and after we launch changes
we can assess and report on success of our design and content improvements and adjust our approach as needed for continuous improvement

**Additional context**

- possible platforms - Chefs, optimal, lime, orbeon
- Consider format of response data - Orbeon delivers data in emailed PDF. Can WP do excel?

**Acceptance Criteria**
- must meet accessibility standards (can't use pop up - survey must be on a separate page)
- choose survey platform
- plain language review of draft survey questions
- check re: Optimal PIA to make sure it covers this type of usage
