Manage BCGW Resource button Missing (DDS-38)

In production the client has a Manage BCGW button, this then pops up and asks a few questions that they need to identify and then it creates the resource with the required fields.

In Beta to get the "Request Access" (see issue #364 regarding wording for this) button to appear, one has to fill in the following:
+ Title: BC Geographic Warehouse Custom Download
+ Resource Storage Location: BC Geographic Warehouse
+ Object Name: Needs to be populated
+ URL: Does this have to be the OFI URL?

This is not intuitive or simple for editors or admin and not ideal as any of these are subject to change by anyone who has access to edit the record.

OFI = True is now back as a field and could be utilized.

Compare the production Manage BCGW flow with the beta Request Access behavior described here, and review issue #364 for the related wording. Identify how the required questions and OFI field should drive resource creation; done means editors and admins can use the button without manually maintaining record fields.
