Document upload modal reminder banner

As an ALC staff member, I often need to be reminded to add the date and properly deal with my attachments before uploading a file to ALCS.

**Acceptance Criteria**

- Add a reminder banner to the following document upload modals in ALCS
- Application > Document tab
- NOI > Document tab
- SRW > Document tab
- Planning Review > Document tab
- Inquiry > Document tab
- C&E > Everywhere
- Reminder, do not add to the Decision document modal in Applications, NOIs, and Planning Reviews

Start with the ALCS upload modal components for the Application, NOI, SRW, Planning Review, Inquiry, and C&E document workflows, using the linked Figma design as the visual reference. Check how the Decision document modals are distinguished, since they must be excluded. Done means the reminder appears in every listed workflow and nowhere in the excluded Decision modals.
