_I want_ to receive a notification when an Institution Offering Change Request is created
_So that_ it lessens the workload of manually reviewing them.
### Acceptance criteria
An offering change request is submitted
- [ ] GC Notify sends a notification e-mail to designat@gov.bc.ca
- [ ] New template required in GC Notify
