Report creation: Cancellations at a location by program year

As an institution user I want to be able to access a report that shows all cancellations that occurred at my location by program year so that I can better deal with my data clean up processes.

- [ ] Create a report that shows the following data
- [ ] Student name, application number, cancellation date, TBD
- [ ] Search criteria
- [ ] Location
- [ ] program year (defined by August 1st 20xx to July 31st 20yy for program year 20xx-20yy) create pre made values till program year 2098-2099. Open to better technical solution
