Add unique identifier to PIR, COE and Report a Change pages

**Describe the task**
As an institution user, I want to have unique identifiers added to the COE: View Financial Aid Application page, Program Info Requests: View Application page, and the Report a Change: View application page so that I can identify students who may have the same name (e.g. 10 Minnie Mouse's)

NOTE: This might be resolved with reports. Privacy issues with DOB, SIN on particular pages?

-SIN is not required when student is applying to an institution

Clarify the required identifier and privacy constraints around DOB and SIN before changing the views. Done means each page provides an agreed unique identifier without exposing prohibited personal data, with verification for all three pages.
