Exception decision notes field

As a ministry user i want a text box attached to each exception so that i can provide decision and next step information directly in sims.

**Acceptance Criteria**
- [ ] Create an open text field under each individual decision for an exception.
- [ ] Read only view of this decision would be visible for all user types that can access student data (min, student, and bc public).
- [ ] Auto approval would NOT need to persist this information in future exceptions.
- [ ] Details on size restraint for size of box and how much content can be include tbd.
- [ ] Current note box would still exist and behave the same. The new fields are each decision specific.

- Request from service delivery to stream line the slowest portion of the exception process which is letters
