Add picklist/dropdown for Country and Province/State

**Describe the task**
Add the picklist/dropdown lists for Country and Province/State for Supporting User Portal and Student Portal (Student Declared Parent information) for consistency and ease of use.

**Acceptance Criteria**

Add to Supporting User portal: Parent Declare
Add to Supporting User portal: Partner Declare
Add to Student portal: Student declare>parent information

- [ ] Add "Country" dropdown list with "Canada" and "Other"
- [ ] Add "Province" dropdown list with all provinces and territories
- [ ] If standard dropdown to have highest used selection first, place "British Columbia" as the first option, then Alberta and remaining in alphabetical order: "Alberta", "Manitoba", "New Brunswick", "Newfoundland and Labrador", "Northwest Territories", "Nova Scotia", "Nunavut", "Ontario", "Prince Edward Island", "Quebec", "Saskatchewan", "Yukon" OR have all in alphabetical order: "Alberta", "British Columbia", "Manitoba", "New Brunswick", "Newfoundland and Labrador", "Northwest Territories", "Nova Scotia", "Nunavut", "Ontario", "Prince Edward Island", "Quebec", "Saskatchewan", "Yukon"
