Description
**As a** *institution user* **I want** *the notes feature for unsuccessful completion "other" to have a warning that this will be seen by students and SABC* **So That** *I make sure that I do not put any negative comments*
**As a** *institution user* **I want** *coding ability removed from the field* **So That** *I do not accidentally provide unwarranted formatting*
### Acceptance Criteria
- [ ] Add note below the "Notes" long text entry field with "This note is visible to student and StudentAid BC staff."
- [ ] Remove the ability to add code in the text field
