Ministry: Add COE declined date

As a Ministry user I want to see the date enrolment was declined to assist with funding after end date exceptions.

Both PT and FT
**Acceptance Criteria**

- [ ] On the Ministry user portal
- [ ] Display the date the student enrolment was declined by the school on the Funding Summary page.
- [ ] Format date: "Month DD, YYYY" Ex: April 4, 2025

Overview:
- If a school declines enrolment, we’d like a reference to show when it was declined.
- Having reference as to when it was declined would allow Ministry staff to better understand the timeline when researching an appeal (re: FAED).

- Impacts Ministry – we can determine if/when student was alerted their COE was declined and better asses if/how it impacted their ability to make appropriate changes in time.
