Enhancement - Auto Add Application Number to Notes

**As a ministry user I want all notes to have an application number when applicable. This is important for being able to identify where restrictions were applied or to what application work was done.

**Acceptance Criteria**
- [ ] Add "Application #" under the date in the notes UI for each time a note is added if applicable.
- [ ] If there is no application number attached then leave it blank and do not populate either new item

**Additional Context**
Lower priority for manually entered notes as they can add application numbers to their note requirements
This is higher priority for system created notes or notifications such as restrictions
