Designation Request - Add Institution Location Code

**User Story**
As a ministry user, when I'm reviewing a designation request a key piece of information I need is the location code. Institutions can only have one location per valid federal institution code.

**Acceptance Criteria**
- [ ] Add a "Location code" field between Name and Address.

**Business Context**

Locate the designation request review screen and find the existing Name and Address fields. Start by reading how those fields are rendered and how designation request data is provided. Done means a Location code field appears between Name and Address for ministry users.
