Render Student Course(s) Viewable through and COE form

**User Story**
As an Institution user, I want to be able to see a student's self-identified list of courses so that I can accurately complete their Program Information Request, especially when I'm not a B.C. Public school and therefore don't have any other access to student info

**Acceptance Criteria:**
- [ ] Add students course(s) to the COE form (similar to what exists for PIR already)
- [ ] Convert page to vuetify? vue? from formio (should fix the strange display for study breaks when there are none)
- [ ]

**Business Context:**
- This is important for PSIs to enforce policy relating to repeats courses etc...
- must add caveat that we do not validate and are not the source of truth for what courses a student is registered to so isn't must still review.
