Support Portal: Order of fields in Review you profile to be in logical order (e.g. Given names, last name, date of birth)

**User Story**
As a _support user_, I want the questions reviewed so that I provide only the most important information to the Ministry.

### **Acceptance Criteria**

Supporting information section

Review your profile section

- [ ] Move "Last name" field between "Given names" and "Date of Birth" field

When screen is minimized, the last name field will drop below date of birth field
