Re-Order Name Fields and Address Fields on Student Profile

**Story Description:**
As a student who logs in with BCeID, I want to input my information in a sequence which is 1) intuitive, and 2) consistent with the sequencing of other areas of the SIMS application, so that I don't input information in the wrong field accidentally and get locked into incorrect information.

**Acceptance Criteria:**
- [ ] Re-order Given Name and Last name field on the profile
- [ ] Re-order address fields on the profile

Start at the student profile entry point and inspect how the Given Name, Last Name, and address fields are currently rendered. Compare their sequence with other areas of the SIMS application, as requested in the story. Done means the name and address fields follow the intended consistent order and both acceptance criteria are satisfied.
