As a student, I want to select the appropriate document purpose so that it is clear to the Ministry user who reviews my information.

**Acceptance Criteria**
1. - [ ] Update _Document purpose*_ dropdown list with new content and remove one item.

Identity

- Add “Disability”

- Update current text “Appeal” to “Appeals”

- Remove “Identity”
