Require Supporting Information when End Dates are Past

**Story Description**
As a student, I want to be alerted to the need to upload supporting information anytime I apply for a Study Period Offering with an end date that is already in the past.

**Acceptance Criteria:**
- [ ] When student selects Study Period Offering with end date in the past, add supporting documentation uploader similar to other exception requests.
- [ ] When student selects my offering isn't listed or my program isn't listed and there is free form dates. We need there to be a validation that checks if current date is past the end date they under if yes it needs to trigger the same uploader as first AC.

**Mock-Ups**
- Need to create mock-up with the proposed text for the document uploader and list of acceptable requirements.
