- [ ] Overawards balance

**Acceptance Criteria**
- [ ] Include text at top of all pages (listed above) to indicate all fields marked with a star "*" are mandatory;
- [ ] Include instructions on how to type phone numbers, dates, and any elements which may have variable formats for all pages listed above.
