Create Program in the Institution Portal has dropdowns without the appropriate dropdown arrow symbol and the asterisk shows up in the dropdown rather than beside the question

**Expected Behaviour**
Dropdowns should display the appropriate dropdown symbol and the asterisk should display next to the question rather than in the dropdown

**Actual Behaviour**
Dropdown shows up without the appropriate dropdown arrow symbol and the asterisk shows up in the dropdown rather than beside the question


** Steps To Reproduce**
1) Login as a Institution user and click on any Location and select Program. 2) Then click on Create Program
3) The form dropdowns show up without the appropriate dropdown arrow symbol and the asterisk shows up in the dropdown rather than beside the question

Reproduce the issue by logging in as an Institution user, opening a Location, selecting Program, and choosing Create Program. Inspect the form's dropdown and required-field rendering; the work is done when each dropdown has the appropriate arrow and the required-field asterisk appears beside the question rather than inside the dropdown.
