Varimathras and other NPCs are not spawned pre-Battle for the Undercity bracket.

Before the Battle for the Undercity was released, many NPCs were placed in the Undercity (such as Varimathras) or were in place of other mobs (like the Kor'kron Elites). These should be spawned in the brackets prior to the Battle for the Undercity quest is available.

Start by locating the progression brackets and NPC spawn definitions for Undercity, then compare the pre-Battle for the Undercity state with the current spawns. Check the reported cases, including Varimathras and the Kor'kron Elites, and verify that the expected NPCs appear before the Battle for the Undercity quest becomes available.
