[Bug]: intent-statement.md  is still not recognized as saved

Description

The AIDLC flow stuck in 'intent-statement.md' is still not recognized as saved' even though the file has been saved manually. It also gives error on 'PreToolUse'

Steps to Reproduce

1. Clone AI-DLC Workflow v2.7.0
2. Setup aidlc in project using instructions from repo README.md.
3. Make sure '/aidlc --doctor' pass
4. Run '/aidlc '
5. Continue along the flow till it ask to review and save 'intent-statement.md' file
6. Save the review and continue in terminal.
7. The terminal will give error with cannot detect save.

Expected Behavior

AiDLC workflow should work properly.

Reproduce the flow with GitHub Copilot CLI 1.0.83 on the reported environment. Done means a manually saved intent-statement.md is recognized and the PreToolUse error no longer blocks progress.
