Description

## Problem
When setting a tool to Always Allow the chat is still requesting permission to run.

## Steps to reproduce the issue

1. Go to 'MCP server tools settings.'
2. Click on 'Always allow'
3. Go back to chat and request to execute tool
4. Q still asks permission to execute tool.

## Expected behavior
Q should execute without asking for permission.

## System details (run `AWS: About` and/or `Amazon Q: About`)

OS: Windows_NT x64 10.0.26100
Visual Studio Code extension host: 1.105.1
Amazon Q: 1.102.0
