Description
## Problem
In Windows VS Code, the Code Issues column of the **Amazon Q extension** continues to show issues even after they are resolved.
## Steps to reproduce the issue
1. Open a project in VS Code with Amazon Q enabled.
2. Trigger an issue so it appears in the **Code Issues** column.
3. Fix/resolve the issue in the code.
4. Observe that the issue still persists in the Code Issues list, even though it is no longer valid.
## Expected behavior
The Code Issues column should update in real-time (or after a refresh) and remove resolved issues automatically once they are fixed.
## System details
- OS: Windows 11
- Visual Studio Code version: 1.103.1
- AWS Toolkit version: 3.71.0
- Amazon Q version: 1.89.0

Research direction
Reproduce the issue in Windows 11 with VS Code 1.103.1, AWS Toolkit 3.71.0, and Amazon Q 1.89.0, then trace the Amazon Q Code Issues column's update or refresh path. Done means resolved issues disappear from the list automatically after the underlying code is fixed or after a refresh.
