Error with code issue window

## Problem
/review agent takes a long time to review the code. Sometime it doesn't produce anything . and sometimes it runs and give me output of vulnerabilities int the chat window and doesn't update the code issues

- OS: macos
- Visual Studio Code version: 1.102.3
- AWS Toolkit version: aws-cli/2.27.31 Python/3.13.4 Darwin/24.6.0 source/arm64
- Amazon Q version: 1.13.3

Start in the IDE's Q chat by running `/review @workspace` on macOS with the listed VS Code, AWS Toolkit, and Amazon Q versions. Observe whether the review produces vulnerability output without updating the code issues window, or produces no output after a long delay. Done means the review completes reliably and its findings update the code issues window as expected.
