## Problem
Starting with Amazon Q 1.63.0 I have been unable to get it to read any open in-progress document in VSCode
## Steps to reproduce the issue
1. Upgrade the Q extension to any version greater than 1.62.0
2. Create a new document
3. Enter some content
4. In a Q chat within VSCode, ask it to read/explain/process what is in the open editor
## Expected behavior
Expected behavior is to read the editor and do what I ask it. This worked prior to the Q 1.63.0 update.
## System details (run `AWS: About` and/or `Amazon Q: About`)
- OS: ZorinOS 17.3 (but also tested on Linux Mint, and Mac OS)
- Visual Studio Code version: 1.101.2
- AWS Toolkit version: not installed
- Amazon Q version: 1.63.0 and every version since then
