MacOS

### Expected behaviour

When Q CLI is asking for permission to run a tool (y/n/t) and I answer with `/clear` I expect the tool use to be aborted.

### Actual behaviour

After `/clear` the permission prompt is repeated.

### Steps to reproduce

- `q chat`
- Ask for something that Q needs to ask permission for. `Write a haiku about permissoins in the file "haiku.md" in the current directory"
- You should get: "Allow this action? Use 't' to trust (always allow) this tool for the session. [y/n/t]:"
- Respond with `/clear`
- The permission prompt is repeated instead of aborted
