Stream tool deltas

**Current Behavior**
When waiting for a tool response, the Q CLI indicates that it is "Thinking...". We do not provide any indication of the contents of the tool change until the entire tool response has been sent. This can result in long pauses when large files are being written.

**Expected Behavior**
Tool calls (particularly for filewrite) will stream the changes, so the user can see what the model is doing while it is "thinking"
