Add streaming support for tool responses in chat UI

### Operating system

macOS 15.3.0 (24D60)

### Expected behaviour

Tool responses would stream in real-time in the chat UI, similar to how regular text responses stream. This would provide immediate feedback when running commands or using tools that take time to complete.

### Actual behaviour

Currently, tool responses only appear after the entire operation is complete, which can make the UI feel unresponsive during longer operations.
